Enrollment Process

Follow these steps to complete your admission

College Department

Step 1: Submit the following to the Registrar's Office through e-mail.

  • Accomplished Application Forms
  • Accomplished Recommendation Forms
    1. Pastor and District Superintendent
    2. Lay leaders of the local church
  • SF9 (Formerly Form 138-Report Card) or Certificate of Transfer Credential with TOR for Evaluation
  • Good Moral Certificate/Transfer Credential Certificate
  • PSA Birth Certificate (Photo Copy $ Original)
  • Medical Certificate(Should indicate health restriction or medication if any)
  • 2 Copies of 2'x2' photos

Step 2: After evaluation you will receive an 'Acceptance Letter' throught e-mail to be presented during the enrollment process.

Your documents will be reviewed by the admissions office.

Step 3: Proceed to Enrollment Process

  • Enrollment Fee - P1,500.00

Basic Education (Preschool - JHS)

Step 1: Submit Requirements

  • • Form 138 (Report Card)
  • • PSA Birth Certificate
  • • Good Moral Certificate
  • • 2x2 ID Picture (2 copies)
  • • Barangay Clearance

Step 2: Assessment

Student records will be checked and evaluated.

Step 3: Payment

Pay the required fees at the cashier.

Step 4: Enrollment Confirmation

Student is officially enrolled and assigned to a class.
